New Employee Forms

This page includes forms for both W2 (Managers) and Trainers. All forms must be completed and sent to the corporate office. Trainers are REQUIRED to have current CPR Certification and Liability Insurance naming Bailey's Gym Inc and SweatPT, LLC, as additionally insured before they are allowed to work.
